The purpose of this study was to investigate the recruitment of the paraspinal muscles longissimus and iliocostalis during dynamic contractions of superman exercises on stable and unstable surfaces, and during unloaded bodyweight squats. Surface electromyography (EMG) was measured from these two muscles bilaterally while participants performed the exercises. Participants (n = 17, age 24 ± 3.1 years, body mass 78.5 ± 8.2 kg, height 1.81 ± 7.4 m) performed two sets of 10 repetitions for each exercise in a random order. Correlation analysis demonstrated a strong positive relation between both sides of the participants (longissimus pairs: r = 0.945; and iliocostalis pairs: r = 0.855), demonstrating a symmetric muscle recruitment in all exercises. However, there were no significant differences between the exercises for both muscles in a multiple comparison analysis (longissimus pairs: p = 0.496; and iliocostalis pairs: p = 0.296). The bodyweight squat produces similar muscle activity of the longissimus and iliocostalis compared with superman exercises on stable and unstable surfaces. Considering the dynamic nature of the bodyweight squat, and its implications on functional daily living activities and sports conditions, it seems a more reasonable choice for targeting paraspinal muscles if compared to superman exercises or unstable superman exercises. “Knee Joint Osteoarthritis” is the topic chosen for the first editorial of the second volume of this journal. The aim of this editorial is to discuss this interesting but little analyzed topic in the current literature, in order to explain and help readers to better understand the functional anatomical aspects of knee joints affected by Osteoarthritis (OA). As the knee joint is tricompartmental, numerous radiographic patterns of disease are possible and the differences between the two main compartments of the knee (patellofemoral joint versus tibiofemoral joint) are explored in this editorial. The hand represents one of the most remarkable expressions of humanization of the anterior limb. The anterior limb, at first ambulatory, underwent continuous evolution acquiring innumerable new functions. In the course of human evolution the hand has undergone continual structural and functional adaptations, characterized, among others, by enrichment of peripheral innervation and further development of the thumb. This development was accompanied by important changes in the brain and the relocation of the eyes, together allowing the muscle control and stereoscopic vision, necessary for a controlled grip. The anatomy of the hand is complex, intricate, and fascinating. Its integrity is absolutely essential for our everyday functional living. It is intimately correlated with the brain, both in the evolution of the species and in the development of the individual. Actually, we can state that we “think” and “feel” with our hands, hence, their contribution is essential to the mental processes of thought and feeling. The aim of this review is to evaluate the most typical hand quality, the prehensility and hence, the possibility of manoeuvring tools. Our attention is mainly focused on the hand anatomy and prehensility during pushing and pulling motions. In particular, our attention is directed toward the relationship existing between the hand prehensility and the volume of the object to be gripped. As an example, we use a grip of the paddle and, pushing and pulling motions during kayak paddling. Indeed, we are firmly convinced that the prehensility plays a crucial role not only in performing the stylistically correct paddling, but especially in realizing a more effective and powerful paddle stroke. This review highlights a great link existing between biomechanical and anatomical notions and sporting performance. Full article

This study aimed to investigate the effect of trunk extensor muscles fatigue, through a submaximum and time-limited isometric task, on the postural control and sensorimotor integration of young healthy adults. Previously and after a submaximum and time-limited isometric fatigue protocol of trunk extension muscles, 30 s stabilometric recordings were taken in both conditions: eyes opened and closed. The center of pressure (CoP) displacement in the anteroposterior (AP) and mediolateral (ML) directions, as well as the total displacement (TD) of CoP were analyzed from the recordings with both conditions (eyes opened and closed) and moments (PRE and POST fatigue protocol). Additionally, the Romberg Index was calculated for CoP displacement in the AP and ML directions, as well as for TD PRE and POST fatigue protocol. Significant differences between the studied parameters of pre- and post-fatigue protocol were not observed. The applied fatigue protocol was not able to modify the postural control, as well as the capacity of integrating sensorial information in the absence of vision, of young healthy subjects. These results indicate that proprioceptive information remains reliable after the used fatigue protocol, allowing subjects to keep a satisfactory straight posture. Full article

The goal of cancer treatment is to arrest or eradicate the tumor while minimizing the often toxic effects of treatment. While oncologic diagnostics and treatments are improving and survival rates are increasing, it is critical to provide additional patient management that appears to affect long-term survival. Epidemiologic evidence suggests patients diagnosed with cancer tend to fair better after treatment when following a comprehensive rehabilitation program aimed at achieving an ideal body weight through dietary and exercise interventions while also increasing cardiorespiratory fitness, strength, mobility, neuromuscular integrity and psycho-social wellbeing. Additionally, it is now being tested whether exercise during cancer treatment and possibly prior to the start of treatment may increase post treatment outcomes by altering body weight, having direct effects on tumorigenesis, reducing oxidative stress and inflammation, and perhaps increasing chemotherapy delivery efficacy. The purpose of this review is to outline an evidence based model to evaluate cancer patients and provide guidelines for post-cancer treatment rehabilitation programs. Additionally, strategies aimed at changing lifestyle habits such as smoking and poor dietary habits will be addressed. Full article

Stem cells have shown promise as a therapeutic intervention by enhancing skeletal muscle regeneration following muscle injury. The purpose of this study was to determine the effect of mouse mesenchymal stem cells (MSCs) on muscle function following a single stretch injury in the calf muscle of C57BL/67 mice. A custom isokinetic device was used to induce a single stretch injury. An intramuscular injection of MSCs or saline was administered three days post-injury. Mechanical testing to measure peak isometric joint torque in vivo was done immediately and at seven or 14 days post-injury. Susceptibility to reinjury was assessed in the soleus muscle using an in situ repeated eccentric contraction (ECC) protocol. In vivo isometric torque of the plantar flexors dropped immediately following stretch injury by 50%. Treatment with MSCs attenuated the torque deficit at seven days, while there were no differences in torque deficit between groups at 14 days. In situ ECC testing of the soleus showed a significant specific force drop following injury, with the MSC group demonstrating a protective effect at seven and 14 days. These results demonstrate transient improvement in isometric torque and reduced susceptibility to reinjury following single stretch injury with intramuscular injection of MSCs. Full article

Estimating the knee functional axis (KFA) is crucial to both correctly implanting a prosthesis and accessing joint kinematics. Researchers have reported on the KFA mainly with regard to manual management of flexion–extension movements performed passively without any voluntary movements. Active touch and movement refers to what is ordinarily called touching—variations in skin stimulation caused by surfaces are altered together by variations in motor activity. This fact is worth noting because it is often neglected that locomotion and its surfaces form an inseparable pair. The objective of this study was to confirm the distinction between touching and being touched. We were particularly interested in measuring the instantaneous axes of the knee (IAK) during locomotion. We propose the invariant structure of the knee as an attempt to distill a kind of higher-order simplification of a knee model relative to the enormous complexities of analytical anatomical objects. The purpose of this study was to evaluate the relationship between spine alignment and shoulder elevation in a kyphotic elderly population. Twenty-three consecutive female patients who underwent balloon kyphoplasty for spinal compression fractures without shoulder symptoms were examined (mean age, 76 ± 6.3 years). Thoracic kyphosis angle (TK), lumbar lordosis angle (LL), and pelvic tilt angle (PT) were measured on standing true lateral radiographs of the whole spine. The shoulder active range of motion with or without scapular fixation was measured in the sitting position. The University of California Los Angeles (UCLA) Shoulder Score was recorded as the functional evaluation. The total elevation angle was 128.9° ± 22°, the glenohumeral angle was 80.7° ± 10.9°, and the UCLA Shoulder Score was 32.4 ± 2.1. The TK was 36.7°, the LL was −29.1°, and the PT was 30.5°. The total elevation angle was significantly negatively correlated with LL. TK and PT were not significantly correlated with any parameter. The total shoulder elevation angle was limited in the kyphotic elderly women in this study, and kyphotic change appeared to interfere with spinal extension and reduced the total elevation arc with no glenohumeral motion deficit. A reduction in total shoulder elevation was negatively correlated with LL. Full article

The purpose of this study was to assess changes in the thickness of the retrocalcaneal bursa (RB) with respect to the ankle position with or without flexor hallucis longus (FHL) contraction. Forty feet of 20 female volunteers were examined. RB was sonographically scanned as the horizontal short-axis image of the posteromedial talar process and the calcaneal tuberosity. RB thickness was measured at three different ankle positions (20° dorsiflexion, neutral, and 40° plantar flexion) with or without FHL contraction. Without FHL contraction, the mean thickness of the RB was 2.6 mm in dorsiflexion, 2.4 mm in neutral, and 2.3 mm in plantar flexion. With FHL contraction, the mean thickness of the RB was 2.5 mm in dorsiflexion, 2.5 mm in neutral, and 2.3 mm in plantar flexion. No significant differences were detected with respect to the ankle position or FHL contraction. This study showed that in healthy subjects, the thickness of the RB in the horizontal plane did not change with respect to the ankle position with or without FHL contraction. Full article

The aim of this study was to investigate the effects of kinesio taping (KT) in combination with moderate adapted exercise on the functional outcome of patients with knee osteoarthritis. We included 66 patients who presented pain and functional impairment with osteoarthritis. At the end of the treatment, only 57 patients remained because nine patients were excluded. Patients were randomly allocated to three treatment groups: (1) exercise group; (2) exercise KT with tension application (stabilizing effect) group; (3) exercise KT without tension application (draining effect) group. We used different methods of investigation: the Western Ontario and McMaster Universities Arthritis Index, the Visual Analogue Scaling Score for Pain, the Timed Up and Go test and analgesic consumption. Our results showed a reduction in knee pain, improvement in knee function and also less need for medication in patients with knee osteoarthritis treated with kinesio taping in combination with exercise for, at least, a period of three months. In conclusion, we can assert that therapeutic knee kinesio taping in association with a moderate adapted training is an effective method for the management of pain and disability limitations in patients with knee osteoarthritis. Echocardiography has recently undergone innovations due to the availability of deformation parameters as strain, strain rate, torsion and rotation that allow an accurate assessment of myocardial function. Because of this general progress, the importance of myocardial deformation parameters has been highlighted, and some aspects of their clinical and research applications have recently been considered for the daily management of many acute and chronic metabolic diseases. The deformation parameters are largely proposed for the early detection of myocardial dysfunction, especially in the case of patients being completely asymptomatic. Strain analysis is extensively applied to cardiomyopathies, to coronary artery disease, or to the evaluation of the “forgotten chambers”, such as the right ventricles and atria. More recently, several other clinical contexts, like non-communicable chronic diseases (NCCD), have actually been benefitting from specific evaluation by strain analysis. Lately, some specific aspects of strain evaluation, particularly Global Longitudinal Strain (GLS) have been shown to provide useful information of clinical relevance in the case of cancer patients. This paper presents an initial review of the recent applications of strain analysis in cardio-oncology, in order to share the recent experience in this field and to support the role of these parameters in cardio-oncology. Electrical stimulation (ES) and magnetic stimulation (MS), applied peripherally, may be used to elicit muscle contractions to increase muscle hypertrophy, increase muscle strength and reduce knee laxity in rehabilitation following injury. We aimed to examine the effect of a three-week exercise programme designed to induce muscle hypertrophy augmented by peripheral ES and MS. We hypothesised that the use of peripheral stimulation to augment voluntary drive during a resistance-training protocol would induce more repetitions thus leading to increased thigh circumference, muscle layer thickness, and quadriceps strength whilst decreasing knee laxity. Thirty healthy participants were divided randomly into either ES, MS or Control groups. Five resistance training sessions were carried out, consisting of four sets of quadriceps extensions. During the first three sets the participants performed eight repetitions at 85% of their 1-repetition maximum (1-RM). On the last set, the participants were instructed to perform the exercise until failure. The augmentation of peripheral stimuli allowed the MS and ES groups to continue to exercise producing, on average, 4 ± 2 and 7 ± 6 additional repetitions with ES and MS, respectively. Following the training, significant increases were observed for both 1-RM (p = 0.005) and muscle layer thickness (p = 0.031) whilst no change was observed in thigh circumference (p = 0.365). Knee laxity decreased (p = 0.005). However, there were no significant differences in the stimulation groups compared with control for any of these measurements. The additional repetitions elicited by stimulation after the point of failure suggests that peripheral electrical and/or magnetic stimulation may be useful as an adjunct for resistance training. However, this effect of resistance training augmented by peripheral stimulation on hypertrophy, strength and knee laxity may be small. Full article

Quality of life (QoL) is one of the most important factors for cancer patients related to survival. Literature supports the role of physical exercise as therapy for improving QoL in this category. This study aims to verify the impact of an unsupervised physical activity program at moderate intensity in this field. Fourteen breast and colon cancer patients, aged 63 ± 15, were prescribed a program of unsupervised exercise for at least six months. They were administered the SF36 questionnaire at the beginning of the program and after six months, to evaluate QoL parameters. Significant improvements in PF (physical functioning) and RP (role physical) (T0: 78.9; T6: 79.2; T0: 53.6; T6: 73.3), mental health role (T0: 54.6 ± 19.53; T6: 55.7) and RE (role emotional) (T0: 54.7; T6: 73.8), with a decrease particularly in pain perception (T0: 21.8; T6: 17.1), were found. A significant decrease in the intake of calories was also evident. In this study unsupervised exercise programs were seen to significantly improve QoL perception in a short time, considerably decreasing metabolic risk factors. Full article

Despite the common use of jogging strollers, only a few studies cover their influence on running patterns. Kinematic studies have shown differences in the upper body movement, while potential changes in plantar pressure remain unclear. Therefore, 30 healthy sport students were asked to run at a self-selected speed with and without a jogging stroller on a treadmill. Via bilateral insole pressure measurements, the plantar pressure distribution and the stride length were quantified. A significantly decreased pressure for running with a jogging stroller was found in the forefoot, metatarsus, inner edge, outer edge and for the total pressure of the left foot. The right foot showed similar significances for all foot areas except the metatarsus. Stride length was significantly shorter when running with a jogging stroller. As stride length and plantar pressure are known to correlate, the reduced stride length in running with a jogging stroller can partially explain the measured decrease in plantar pressure. Further, the partial weight-bearing, which is needed to keep the jogging stroller in line, can also contribute to the decrease in plantar pressure. Moreover, the unilateral weight-bearing enlarges the base of support and could also be a reason for the decrease in plantar pressure. In conclusion, our results show statistically significant changes in plantar pressure, but these are not of clinical importance as the effect sizes are very low. Therefore, no specific training recommendations for running with a stroller have to be made. Full article
