    In the end, our goal was to provoke a discussion about the role of RCTs in improving patient outcomes, and have some fun while we're at it. And judging by the response we've received, we'll consider this a win. [Mic dropped, without parachute]

    Like all RCTs, the findings will be misquoted and misinterpreted by some. But that's hardly a criticism of the trial itself. Though we probably owe our readers an apology for all those parachute jokes that don't land.

    So no, we're not lampooning RCTs. Or making fun of evidence-based medicine (or anyone, other than ourselves). And we are certainly not making the case for medical parachutes.

    So what's the work around? Ironically, it's performing rigorous RCTs *before* strong convictions about effectiveness or safety become entrenched. And always interpreting RCT results in context. For you visual learners, here's a cue to why context matters.

    The fact that there are very few (if any) parachutes in medicine doesn't alter this phenomenon. IT'S NOT THE PRESENCE OF PARACHUTES BUT OUR BELIEF IN THEM THAT BIASES THESE RCTS.

    But conceivably, if there were strong pre-existing notions about the adverse effects of an intervention, patients at highest risk of harm would not enroll, and said RCT may miss a clinically important safety signal. It cuts both ways.

    In our example, strong beliefs about the effectiveness of a parachute meant that patients most likely to benefit didn't enroll in the RCT, which biased the results to the null.

    There's also a more nuanced take-home. When clinicians have strong pre-existing convictions about the effectiveness or safety of interventions, it affects the kinds of patients enrolled in an RCT. Importantly, applies even if the underlying convictions are untrue.

    But external validity - whether the results apply to patients *not* enrolled in the trial - is less straightforward. Do the jumps in your practice look like those in the trial? RCTs are informative but they are not magical: their results should not engender a blind leap of faith.

    We offer 2 take-home messages, one more nuanced than the other. First, RCTs sit atop the evidentiary hierarchy because they are the champions of internal validity - randomization is a very effective antidote to confounding.

    NEW in . PCIs coded for unstable angina in the outpatient setting ⬆ from 2010 to 2014. This ⬆ was most dramatic in NY (14x). Have diagnostic/coding patterns changed in response to efforts to reduce inappropriate PCIs?

    My take-aways from the awesome 👏 PARACHUTE trial by and et al: RCTs are 100% necessary, bias is very real, critical appraisal of literature mandates digging deep beyond titles and abstracts, and...humor is vital to our survival...
