Objectives of Thailand’s Visit

The USTDA Reverse Trade Missions aim to bring international project sponsors to the United States, introduce them to cutting-edge American technologies, and foster partnerships supporting worldwide infrastructure development.

For Thailand’s delegation, this mission focused on modernizing healthcare through advancements in digital health, diagnostics, and genomics. By engaging with leading American organizations, participants explored innovative solutions, built connections with technology providers, and identified avenues for future collaboration.

Event Highlights

We were honored to be among the select technology providers featured during the Thai delegation's visit. With their expressed interest in enhancing rural healthcare and equity, we were excited to demonstrate how the GALE eClinic platform addresses these priorities.

The highlight of our presentation was a live connection with community health workers in Guyana. Thanks to Verlina Conrado from Paruima and Rexon Jonah from Phillipai, both in Region 7 of Guyana, we \ were able to hear their personal stories and virtually tour the clinic in Paruima, some 4,300+ miles away from the conference, showcasing the power of telemedicine to bridge vast distances. Attendees witnessed diagnostic tools in action, including the EchoNous ultrasound and Mintti stethoscope, as the health workers captured and transmitted a live heartbeat from the Amazon rainforest—broadcast directly into Zoom’s cutting-edge meeting space, the Boardroom of the Future. This experience underscored the GALE eClinic platform’s potential to break geographic barriers and deliver advanced care anywhere in the world.

Innovative Use of GALE eClinics

The GALE eClinics have become a cornerstone of Compass Health Network’s strategy to enhance healthcare delivery. The GALE units are employed across multiple settings, demonstrating their versatility and effectiveness:

Troy Lambert, MHA, BSN, and Vice President of Primary Care Operations, Compass Health Network, commented on how the network continuously finds new ways to integrate the eClinics into their centers:

"Once you get that first use done, you realize you can use it in other ways. It just generates more opportunities. We’ll have a new situation to figure out and say, 'Well, we can't put a provider in that location, but can GALE do this?'"

Behavioral Health and Substance Use Treatment: At Compass Health Network's psychiatric and substance use disorder residential facilities, GALE eClinics are used for conducting health assessments, physical exams, and psychiatry visits. This integration ensures that patients receive comprehensive care despite the challenges of remote locations and limited resources.

Primary Care and Pediatrics: For primary care and pediatric services, especially in rural areas where medical providers are scarce, GALE eClinics bridges the gap by allowing remote consultations and evaluations. This technology ensures continuity of care even when a primary care provider is unavailable due to maternity leave or other reasons.

Enhanced Telehealth Capabilities: The GALE units have significantly improved the network's telehealth capabilities. During the COVID-19 pandemic, the ability to perform remote physical exams, including listening to heart and lung sounds, became crucial. The GALE eClinics facilitated these capabilities, addressing the limitations of traditional telehealth solutions.

"GALE allows us not to miss much. The biggest thing people worry about is, ‘What if I missed this, or what if I missed that?’ But you can still hear the heart and lungs […] so it is much more robust." – Troy Lambert.

Access to Care: Compass Health Network ensures that even the most remote or underserved patients can access quality care by deploying GALE units across various facilities. This is particularly important in rural regions where healthcare providers are limited.

"We're able to deliver care when we wouldn't be able to otherwise, especially for smaller FQHCs that are limited on providers."  – Troy Lambert.

Continuity of Care: GALE eClinics help maintain continuity of care when in-person visits are not feasible. This is crucial for managing chronic conditions and providing consistent follow-up care.

"GALE significantly assists in assessing, starting, and keeping patients on their treatment plans for Hep-C,” Troy adds. “This is particularly noteworthy given that approximately 40% of Hep-C patients are treated through GALE.”

Optimizing Resources: GALE eClinics enables the network to offer comprehensive services without additional physical infrastructure or personnel, optimizing resource use and reducing operational costs.

As Troy Lambert commented, "It's a true force multiplier—it enhances our capabilities without requiring us to be physically present, making our care more comprehensive."

The Genesis of Innovation

As a collaboration between the Mark and Robyn Jones College of Nursing and the Montana Office of Rural Health/Area Health Education Center, an HRSA ANEW grant has provided the Rural Ready Nurse Practitioners training program opportunity for students committed to serving in rural settings. This initiative started just before the onset of COVID-19, with the pandemic necessitating a pivot from in-person learning opportunities to remote education. 

Now that the program has shifted back to in-person learning, telemedicine continues to enrich the curriculum, blending traditional and modern healthcare training methods.

GALE eClinic units offer an array of telehealth tools, enabling students to conduct remote consultations and examinations. The initial reception of these kits during the pandemic was overwhelmingly positive. Undergraduate students were paired with nurse practitioner students to handle standardized patients, creating dynamic, hands-on learning scenarios.

Alicia Crane, DNP, APRN, FNP-C, who participated as a Family Nurse Practitioner student in simulations with undergraduate students, shared her perspective on using GALE in the program:

“It was awesome, and I felt like it was the next best thing to seeing a patient in person.”

Enhancing Learning through Hands-On Experience

The GALE eClinics have significantly impacted student engagement and learning styles, merging well with simulation to accommodate different learning approaches. Some students initially held back and observed, but peer encouragement led to more active participation. The interaction and independent exploration of the technology has enhanced their critical thinking and communication skills.

Students have shown remarkable growth in their competence with telehealth technology. For instance, undergraduate students began asking detailed questions independently and effectively using the available tools. The experience has substantially improved their comfort and proficiency with telehealth technology, preparing them for real-world applications.

Stacy Stellflug, PhD, APRN, FNP-BC, Associate Professor of Nursing at MSU, commented on the undergraduate nursing student’s quick adoption and understanding of the GALE Telemedicine units:

“I thought, ‘That's weird. I mean, they're like brand new. How can they know how to ask all these questions?’  They found the tab (In the GALE platform) with the chief complaint information themselves. It was fun to see them interacting and teaching each other. Initially, we thought we’d have to guide them step-by-step, but they took over and had this fun exchange of information.”

Dr. Alicia Crane commented on the student’s development while using GALE:

“I saw a huge confidence boost in students using these kits. I'm sure even moving to the clinical sites, they built on their confidence and ability to provide care for patients.”

Preparing Students for the Future

Exposure to advanced telehealth tools is crucial for preparing nursing students for their future careers. Students have returned from clinical experiences feeling empowered and ready to demonstrate their newfound skills. Tools like digital stethoscopes have been particularly impactful, with students eagerly sharing tips and insights with nurse practitioners thanks to their previous exposure via their time with the GALE eClinics.

“Learning how to use telehealth and the associated tools provides students with the skill set to serve rural communities where access to healthcare providers is limited,” commented Alicia Crane, “I honestly think they could put it on their resume.”

Integrating GALE eClinic units has fostered a collaborative learning environment that transcends the classroom. Initially piloted with a small local cohort, the program has evolved from a show-and-tell format to a more serious, deeply engaging, hands-on experience. This kind of preparation is critical for students’ future careers.

Alicia Crane continued, “The platform allows for different disciplines to work together, providing learning on how to use a team approach to care for patients. This is beneficial not only for telehealth but also a needed skill for nursing practice in general.”

“These are valuable lessons they’ll carry into their careers and possibly into preceptorship or mentorship roles,” added Stacy Stellflug.

Impact on Rural and Indigenous Communities

The program also lays the groundwork for broader applications in rural healthcare settings. Many undergraduate nursing students and nurse practitioner students come from rural areas, and the program introduces them to technology they can use in their communities. This approach could provide telehealth services to those lacking access to traditional healthcare, including Indigenous communities. The potential for positive impact is immense.

Stellflug highlights the program’s long-term vision of providing telehealth services in rural settings as a training opportunity for students. This involves mentoring experiences where students can go out and provide services, especially in underserved communities.

“We're introducing them to the technology, the tools, and the capabilities they can use with these kits. Perhaps it even means they're out on the reservation doing home visits as nurses, and they can dial into the provider and say, ‘Here we are,’ because the person doesn't have access to or reliable transportation; the healthcare can come to them. This is one facet we're hoping to develop.”

Addressing Healthcare Disparities in Remote Areas

Access to medical services in remote areas remains a significant global challenge due to geographic isolation and limited healthcare infrastructure. According to the World Health Organization (WHO), millions worldwide do not have access to essential health services, and rural communities are disproportionately affected by this gap. Ultrasound technology, a critical tool in medical diagnostics, is often unavailable in these regions due to a lack of trained personnel and resources, leading to delayed or missed diagnoses.

In response to these challenges, 19Labs, in partnership with Northwell Health, has developed an innovative solution to bridge this gap. Northwell Health, known for its expertise and dedication to healthcare innovation, provides the training expertise essential for this program’s success. 

“This goes beyond training for ultrasounds. It's bridging those gaps culturally, too. It brings something into the Hinterland communities and brings something from the communities out. Our Response

Our new online ultrasound training course is designed specifically for CHWs. This course is the first of its kind globally and was launched as part of the National Guyana program. By targeting CHWs, often the primary healthcare providers in remote areas, this training program ensures that vital diagnostic services reach underserved populations and enhances their capacity to deliver quality healthcare in underserved regions.

One key motivation for this program is to empower CHWs with skills that traditionally require extensive formal education and hands-on experience. By leveraging advanced telehealth technologies, the program enables CHWs to perform basic ultrasounds to provide better imaging for specialists to assess, which can be critical for maternal and child health, emergency care, and managing chronic diseases.

Training Course Features

The course is designed to be highly interactive and practical, utilizing the latest teaching methods and technologies. Key features include:

• Practical Training: Remote, virtual, one-on-one, hands-on training sessions with a Northwell sonographer to ensure CHWs gain practical experience. The hands-on training component ensures that participants are not only theoretically proficient but also practically capable of using ultrasound devices in real-world scenarios.

• Innovative Technology: Utilization of the GALE telehealth platform to enhance learning. This platform supports real-time feedback and interactive sessions, making learning more dynamic and engaging.

• Tailored Curriculum: The curriculum focuses on typical conditions and scenarios encountered in remote settings to prepare CHWs to meet the needs of their communities. It includes learning common diagnostic scenarios and terminology commonly used in ultrasounds for better controller understanding and troubleshooting equipment issues.

As Radha said, when seeing the aptitude and enthusiasm the CHWs displayed when receiving the program:

“It's so good to empower these community health workers. It gives them a voice, and I can see how they can excel and teach the other community health workers how to operate ultrasounds over telemedicine. They can be teachers in the future.”

Rexon, a Community Health Worker who participated in the pilot sessions of the training course, expressed his appreciation for it, saying, 

“I really appreciate the people who organized this training because this was my aim to study ultrasound. The training was not difficult. It was good to manage.”

Their involvement ensures the highest training standards, leveraging their ultrasound technology and medical education expertise. Today, the U.S. Trade and Development Agency announced that it has signed a grant agreement with Mexican digital connectivity company Vasanta Comunicaciones SAPI de CV (Vasanta) that will pilot telemedicine technology from California-based 19Labs, Inc. (19Labs) in rural southern Mexico. This feasibility study and pilot project will help develop a roadmap to scale the implementation of high-quality, sustainable telemedicine clinics to 1,000 rural communities across Mexico.

“This project is a great example of the kind of healthcare infrastructure that we want to support. It highlights the value of U.S. innovation, it is transformational, and it will save lives,” said Enoh T. Ebong, USTDA’s Director. “We look forward to seeing how this partnership materializes into greater healthcare access for the people of Mexico.”

The USTDA-funded pilot project will support the deployment of 50 rural telemedicine clinics in the States of Oaxaca and Tabasco. 19Labs will contribute its telemedicine technology, and Vasanta will provide the required digital connectivity for the clinics. The roadmap for scaling the project’s implementation will include a technical and economic analysis, financing plan, and social impact assessment.

The day prior to Ram’s presentation, 19Labs facilitated a remote ultrasound session of a pregnant patient in rural Oaxaca to be streamed in real-time to Jamaica, the location of his discussion. After witnessing this display, many commented on how marvelous it was.

Undoubtedly, it was, but in his discussion, Ram shifted focus to three pillars that allow amazing technology to truly provide solutions in telemedicine.

The primary lesson he discussed may come as a surprise for some.

The three pillars he laid out demonstrated that amazing technology is a must but never, never, never sufficient.

“I've been building products since I’ve been working with Apple, Samsung, Nokia, and multiple startups. So yes, I'm a technologist”, said Ram.

People

Developing the need to focus on people versus technology, Ram said: “Because in the end, it’s all about people. And you need to start to talk much more about the people and less about the technology. And that’s where we’re going to start.

“What do we need to do to make telemedicine work in rural settings?

“Many talk about the doctors, … but I want to take it a step away and toward the community worker who is, more than anything, the key.

“In telemedicine, we connect between two locations. And it’s a connection in which the ‘brain’ is the doctor on one side, and the ‘hands and the heart’ are the community worker. The 18-year-old, usually a young woman, sits over there and holds the hands of the patient.

And yes, you absolutely need the doctors, but if the community worker is not energized and confident, it will not happen."

"Most times, the community workers are the most amazing people I have met.” - Ram Fish

Ram then gave his experience of being inducted into an indigenous tribe on the Brazil/Guyana border called the Wai-Wai during a visit for an eClinic deployment.

Among them was a remarkable community worker named Devina, a young mother of 2. Witnessing how someone like her could bridge the gap between her community and healthcare professionals was truly eye-opening. She was empowered to care for her neighbors, even playing a crucial part in saving a three-month-old baby’s life. This woman, who had rarely left her community, was now empowered to care for her people and had gained access to the world of healthcare and beyond.

“Suddenly, those communities are visible. They are visible in the hearts and minds of the health administration and the minister of health. You bring connectivity, you empower them, and that really changes the story of telemedicine.”, continued Ram.

Leadership

Ram acknowledged a comment made by a presenter at a previous event: the Government must pay. You don’t do a trial if they don’t pay.

Though a valid point, beyond financial support, strong leadership is essential.

Ram described the leadership that makes a successful telemedicine program: "It requires a health minister who says, ‘I want to get this done, and I will remove the obstacles.’ And there will be obstacles, more than you can ever imagine.”

They must consider various aspects: What should be done? How should it be done? Are they sure they want to do it?

It's not just about politics; it's about intentions. Strong leadership is a prerequisite.

Additionally, building strong relationships within communities is crucial. You need leadership individuals in the local communities and educators capable of rallying community support.

Again, drawing on personal experience during a deployment trip, Ram gave the example of a potentially ruined installation and investment. “We checked ahead of time: “Yeah, we have solar power. Everything is ready for you. Come in.” We land. No solar power. How am I supposed to install a satellite with only four hours before the [plane] takes off? I need to get solar power. You figure it out. You get the leadership [to make a call, and] within one hour, we had a generator and a couple of solar panels relocated from the school temporarily. Strong leadership is absolutely a must to make it happen.”

Dashboard

“If you can’t measure it, you don’t know what’s happening. This is how you find problems. This is how you find issues. Quantify what’s happening,” stated Ram when beginning the discussion of the third pillar.

Without the ability to measure progress, you remain in the dark about what's happening. A comprehensive dashboard is a must. It helps identify problems and challenges, allowing you to quantify and understand the situation, leading to necessary adaptations.

Even the best deployments can be improved, but only if there is a way to know where and how the improvement should happen.

Creating an effective feedback loop is fundamental to keep a telemedicine program running efficiently and be of real benefit.

Amazing Technologies Is A Must

Shifting back to technology, Ram called back to the remote ultrasound demo done the day before: “You saw this yesterday, and it looks so smooth and wonderful. But what did we see? You saw video sharing, which is not easy to do. Audio quality you could actually hear, which is not always the case. And you saw it in the condition of rain [from a hurricane in the area] in the community, which usually kills the satellite. So, you had a very low bandwidth connection, and you had it working.”

Breaking down the details and levels of technology involved, Ram listed just some of the components that made the demonstration work.

Drawing attention to the video call and screen sharing used by the GALE platform, he stated: “The technology in here is all based on Zoom. Why? Because I like relying on a company that has spent billions of dollars and is absolutely dedicated to creating the best video conferencing solution.”

“The doctor showed up on time. How did it happen? Because we integrated WhatsApp and notifications from WhatsApp”, continued Ram, focusing on the details that make everything run smoothly.

Now taking it a step beyond smooth operations and into the patient experience, he called out the detail of integrated virtual backgrounds: “You don’t even know that the doctor is at home because there’s a virtual background. And it shows the logo of the Ministry of Health. Now you are the patient who sits over there in the rural community, looks at the doctor and sees a logo, and sees the name of the doctor.”

Speaking of the provider experience during the call, he elaborated on the benefits of the integrated Insights AR feature: “Let’s go on the other side of the screen. You saw what the doctor gets to see. He sits there on Zoom on the tablet, on the phone, or on the computer, and without doing anything else, not another side panel, not another browser, not another tab, all the patient information is displayed in real-time. They look at it for one second and say, ‘Hi Maria,’ and the patient thinks, ‘Wow, he knows my name!’. And the [patient’s] allergies are displayed all the time. So ‘Oh, I’m not going to mess it up’ and give them something they’re allergic to.”

Amazing technology is a prerequisite. It simplifies complex tasks, making them appear seamless and reliable.

Devices

There's been considerable discussion about devices in the telehealth and telemedicine field.

While devices like stethoscopes, blood pressure cuffs, and examination cameras are important, different locations require different devices.

To that point, Ram stated: “You need a deployment management that you control what you put in one community, which is an advanced kit with ultrasound, versus a different community where you have to ship different types of devices because something broke.”

This approach reduces the need for on-site IT personnel, resulting in cost savings.

Beyond devices, consider incorporating hearing and vision tests into the telemedicine platform, especially for rural communities.

Due to the distance and seasonal weather conditions of these Missouri communities, it is common for a provider to be working at one location and unable to travel to others to provide needed consultations.
