That's why we offer Child Health Plus, a New York State sponsored health insurance program for children under the age of 19 who do not qualify for Medicaid. Child Health Plus now covers many more children, giving them great healthcare and the opportunity for a brighter future. And with Affinity, you get access to over 30,000 providers and more than 85 of New York's best hospitals, including those that specialize in helping teens and children.

With Affinity, your child will get access to a full range of health care services and benefits, including:

- Well-child visits
- Prescription drug coverage
- Dental, hearing and vision care
- Immunizations, lab tests and X-rays
To be eligible for Child Health Plus offered through Affinity, children must:
» be under 19 years old;
» live in New York City, or in Nassau, Orange, Rockland, Suffolk or Westchester county;
» not have Medicaid or Medicare; and
» not have any other health insurance.
**Note: children may qualify regardless of their parents' immigration status.**
