Most of these plot holes were actually covered in the movie.
The mission in Lagos was a stealth mission. No one knew the team was there. A big, purple android tends to stick out. Same reason War Machine wasn't there. Plus, I think the mission was to capture Crossbones. Vision is potentially overpowered to do that.
Zemo's plan was just to split up the Avengers. All he wanted to do was send the video to Tony. Heck, he initially didn't even go after Bucky to get the video. When that failed, he used the UN conference to get to Bucky to get the mission report.
Yep, I felt he was getting too negative about everything that was damaged. Though, Tony did mess with tech he didn't understand entirely to create Ultron. If he hadn't, there would have been no Sokovia Accords. Plus, Ross isn't exactly a friend of the Avengers, is he? It's been a while since I watched The Incredible Hulk.
Zemo indirectly triggered the arrival of Iron Man at the facility. He called up the hotel he was staying at which led to the discovery of the actual Doctor and the kit he used to make himself look like Bucky. Iron Man realised his mistake and rushed to help Steve. Plus, I'm not sure how long Zemo actually waited at the facility, since he took a civilian flight to Moscow. From there, he had to get to the facility in Siberia. On the other hand, Captain America had a jet and Iron Man, of course, flew. Worst case, if Iron Man didn't show up, he'd have just got the video to him some other way.
The camera was a tad convenient, yep.
I've never seen a version of Howard Stark with extra security, maybe he just wasn't one of those guys. There are loads of rich guys who don't have extra security though it is odd that he was transporting something from the Pentagon without any backup.
