How to Investigate an Outbreak: And the Band Played On: Epidemiology is the study of the frequency and distribution of disease and other health related factors in a defined population. Other factors included may be virulence of the pathogen, the portals of exit and entry, the course of the disease as well as surveillance. When an outbreak is suspected in an area, the starting point for the investigation usually begins with local health officials. The local health department may request help from the Centers for Disease Control (CDC), which is the principle tracking agency. The steps in an outbreak investigation are as follows: 1) Prepare for Field Work 2) Establish the Existence of Outbreak 3) Verify the Diagnosis 4) Define and Identify Cases 5) Describe and Orient Data 6) Develop Hypothesis link 7) Evaluate the Hypothesis Refine Hypothesis 9) Implement Control and Prevention Measures 10) Communicate Findings We will look at each one of these steps individually to see how a local health agency would begin to track a potential outbreak, to determine if an outbreak is occurring, the putative causative agent, the potential mode of transmission and to suggest possible prevention measures. The local health department would be the starting point for an investigation. As we examine these steps, I will provide some examples from outbreaks we have discussed in class. In each case, I will try to relate the steps from the Esherichia coli outbreak from bagged spinach we discussed earlier in the year as reported in the Morbidity and Mortality Weekly report www.cdc.gov/mmwr/preview/mmwrhtml/mm5538a4.htm
